Incident Narrative

An employee was removing plywood from a forklift truck that was elevated to the third level of the structure (open balcony). As the employee attempted to pull the plywood from the forks, he lost his balance and fell 22 feet to the surface below. The employee suffered broken ribs and a head injury and was hospitalized.
