Incident Narrative
An employee was checking the regrind material level in a machine's regrinding bin. Noticing that the level was high, the employee opened the bin door and saw some handle slugs on top of the material. The employee, believing there was a clog, reached through the bin and into the blender's throat. The low-level sensor became uncovered, causing the slide gate of the blender to close and catch the end of the employee's right middle finger. The employee suffered a fingertip amputation.
