Incident Narrative
An employee was using a hydraulic torque wrench to remove the nuts on a high-pressure frac pump. While removing the last few nuts, the employee placed his hand on the wrench to ensure it did not fall when his other hand contacted the device and it engaged, causing it to rotate and crush the employee's thumb, resulting in a fingertip amputation.
