Incident Narrative

An employee was assembling a hydraulic cylinder in a plant's assembly area. The rod of the cylinder unexpectedly retracted into the tube and the employee's right-hand fingers were trapped in a pinch point between the cylinder tube and the piston rod. The employee's index fingertip was amputated.
