Incident Narrative

On 12/2/2021, at approximately 3:00 p.m., an employee was hanging Christmas lights on the gutter of the company building. He fell roughly 8 feet from the top of an extension ladder and sustained a concussion, six broken bones in his face, and required stitches along the left side of his face.
