At about 2:30 p.m. on November 16, 2021, an employee was checking the sight glass for a liquid level on an ammonia refrigeration system. The sight glass broke, and liquid ammonia sprayed out of the opening onto the employee's face, chest, and throat area, resulting in chemical burns.
