Incident Narrative
An employee was stripping and removing concrete form work. The employee stepped up onto the concrete form work. The metal pole brace used to adjust the wall form was unattached at the top. The employee fell approximately 27 inches and the 12-foot metal pole struck them in the head, resulting in an orbital fracture near their right eye, a fractured nose, and a head laceration.
