Incident Narrative

An employee was using an over the road container (OTR). The employee brought the OTR to a mail dumper, which lifts packages on a conveyor belt. As the dumper was operating it contacted the door of the OTR, causing it to close on the employee's fingers. The employee's left middle and ring fingertips were amputated.
