Incident Narrative
An employee was climbing down a ladder on a ready-mix truck. A lanyard, caught on a bar on the truck, brought him to an abrupt stop; this caused his right foot to catch in the ladder and he fell. He suffered injuries to the neck, lower back, right hip, right knee, and right ankle and lost consciousness.
