An employee was servicing a tractor. He had accessed a tire via a 2-step step ladder. He was stepping down from the tractor's tire and onto the step ladder, but missed the top step of the ladder and fell 3 feet, landing on the ladder and then the ground. The employee was hospitalized with a fractured left rib and lacerated spleen.
