Incident Narrative
Employees were swapping out a fork attachment for a bucket attachment on a loader. The locking pin got stuck. An employee used a piece of rebar to remove debris from the pin receiver; the pin dislodged quickly and his left ring and little fingers were caught between the pin and the receiver. He suffered amputations to both fingers.
