Incident Narrative

An employee was moving a portable plant rack, which was 7 feet high, 2 feet wide, and 4 feet long. The rack began to tip on the uneven ground. It and the potted plants on it fell on the employee, knocking her backward to the ground and causing a possible back injury. She was hospitalized.
