Incident Narrative
Employees were at the facility to perform hurricane cleanup and repair. The frame of a pull-down overhead door had been blown out during the hurricane. An employee was mounting a board to a bracket of the frame to then install a temporary vinyl curtain. The employee was standing on a pallet that was attached to a forklift and was approximately 8-10 feet above the gravel ground. When the employee struck the overhead door, it came loose and struck him, causing him to fall from the pallet to the ground. The employee sustained a fracture to the upper area of the right leg.
