Incident Narrative
At about 3:30 p.m. on September 14, 2021, an employee was riding on the step used to access the cab of a powered industrial vehicle, observing the vehicle's operator for training purposes. The vehicle was traveling through an aisleway of a racking system used to store lengths of steel beams/poles. The employee's right arm struck the steel shelf arms of the racking system resulting in a fractured radius and ulna. The employee was hospitalized.
