Incident Narrative
An employee was changing a belt on a machine as part of preventive maintenance. He put the machine in "Test" mode to make an adjustment to one of the sprockets. A chain caught the employee's left thumb and pulled it into a sprocket, causing an amputation of 2 cm. of the thumb.
