Incident Narrative

On September 10, 2021, at approximately 9:30 p.m., the injured employee was standing in between their powered industrial truck and a pallet of goods stacked approximately 7 feet high. A second employee was traveling through the aisle, pushing three pallets of goods stacked approximately 7 feet high and struck the injured employee's pallet. The pallet struck the injured employee and pinned their left foot between it and the PIV platform and their abdomen against the goods on the pallet and the PIV guardrail. The employee sustained trauma to the left foot and ankle and a fractured ribcage.
