Incident Narrative

An employee was deburring a 1-inch carbon steel pipe on a lathe for weld practice. The employee was polishing the outside diameter of the pipe using emery cloth. The employee was straddling the pipe, with one hand on each end of the cloth, when the rotating pipe pulled in the employee's glove and right hand. The employee's right index finger was amputated at the first knuckle.
