Incident Narrative
An employee was using an emery cloth to polish a round stock bar on a conventional lathe. The shaft grabbed the emery cloth and pulled the employee's right hand against the shaft. He was able to get free, but the shaft then pulled his left hand and lower arm around itself. His forearm was broken.
