Incident Narrative

Two employees were pushing a parts rack through a facility. They stopped to move two vacuum pack hoses that were on the floor blocking the path of travel. While one of the employees was moving the vacuum packs, he stepped on the hoses, lost his balance, fell into a guardrail, and then landed on his right hip. His hip was broken.
