Incident Narrative

Two employees were lifting a 59-pound piece of steel plate into the engine room of a vessel. One of the employees, standing on the ground, lifted the plate up to the other and released it. As the other employee was lifting it into the vessel, it fell down through a hole. The first employee's left ring and middle fingers were caught between the plate's edge and a steel I-beam. Both fingers suffered multiple compound fractures; the little finger and half the ring finger were medically amputated the next day.
