Incident Narrative

An employee was putting a bridge beam in place. When the beam struck the pier, the employee who was holding a 4 x 4 to cushion the beam was knocked off the pier and fell over 40 feet to the ground, resulting in two broken arms and a fractured back that left the employee paralyzed from the waist down. The employee's harness was not connected at the time.
