Incident Narrative
An employee was lying on a concrete slab immediately next to an unloader. The unloader had been raised on a hydraulic jack so the employee could make a shim adjustment to it. When the employee lowered the unloader after making the adjustment, it fell and struck the employee, who suffered a 10-inch horizontal laceration to the chest; the amputation of three fingers on the left hand; a cut to the left side of the head; cuts on the upper right arm; and bruises to the back, left hip, and right shoulder.
