Incident Narrative
An employee was loading pallets of merchandise inside a parked trailer using a forklift. An operator attached his mule to the parked trailer and drove about 10 to 15 feet from the bay deck at the same time as the forklift operator was placing a pallet of merchandise inside the trailer. The employee and the forklift fell to the ground and the employee was hospitalized for a back strain.
