Incident Narrative

At about 7:40 a.m. on July 26, 2021, an employee was settling a bundle of four pieces of square tubing onto a workbench when the tubing shifted and slid onto his left hand and index finger. The finger was pinched and crushed between the tubing and a piece of channel on the workbench, causing an amputation to the fingertip above the nailbed.
