Incident Narrative

The injured employee was standing on the left side of his pallet jack applying stickers to it. A co-worker was operating a pallet jack in the same area. The co-worker was moving to pick up a pallet and the equipment jerked to the left and struck the injured employee's left foot and ankle, pinning them to the pallet jack. The employee's foot and ankle were fractured.
