Incident Narrative

An employee was lowering machinery into a vat of caustic chemical using an electric hoist and crane. An I-bolt (screwed into the heavy machinery and also being held by the hoist) broke, and the machinery dropped into the vat. Caustic splashed the employee's clothing, who suffered burns and was hospitalized.
