Incident Narrative
On July 1, 2021, two employees were filling up a grain truck. One employee was standing in the load out area, which is a platform approximately 15 feet high used to observe the level of grain inside the trucks. Employees access this platform via a ladder that is located above the booth pit. The second employee was conducting groundwork and operating the controls that supply the grain to the trucks located behind the booth pit. During this process, the second employee went to use the stairs to climb up to the loading platform. At this time, the booth pit hole was open and uncovered. While the employee was climbing the ladder, the employee fell approximately 24.5 feet into the booth pit and suffered multiple broken bones and lacerations.
