Incident Narrative
An employee was repairing a wrap line machine. While checking the tension of a chain in the machine, the employee's glove was caught by the chain and his finger was pinched between the chain and a sprocket. His left index finger was medically amputated to the first knuckle. The machine was not locked out at the time of the incident.
