Incident Narrative

An employee was hand digging in a trench 4 feet deep. As the employee was about to exit the trench using a ladder, he tripped on a pipe and fell against a spreader bar with a locking pin sticking out. The employee's lower torso impacted the locking pin when he fell, resulting in a severe bruise and contusion.
