Incident Narrative

An employee was preparing for a concrete pour from the platform of a scaffolding system. The employee stepped on the platform and fell approximately 17.5 feet to the ground along with a scaffold plank. The employee sustained a fractured left wrist, a sprained right wrist, and facial lacerations and was hospitalized.
