An employee removed a piece of plywood to lay down some pipe, then moved to lean the plywood against a wall. The employee fell through a 3-foot by 4-foot hole that had been concealed under the plywood, landing 8 feet below in a crawlspace. The employee suffered a hip injury and was hospitalized.
