Incident Narrative

Employees were moving an oil rig's horse head out of the way so they could install a blowout preventer. As the operator picked up the winch lines, the two floor hands began moving the horse head to the side. The horse head flipped over, and an employee's finger was caught in the winch line and the horse head's bridle cable. The finger was lacerated and the fingertip was amputated above the nailbed.
