Employees were disassembling a flange test adapter on equipment after it had undergone hydrostatic pressure testing. The injured employee was loosening the bolts holding the test flange to the equipment when pressurized water (22,000 psi) escaped and struck his upper legs, causing severe lacerations to both thighs. The employee was hospitalized.
