Incident Narrative
On June 2, 2021, at 8:20 a.m., an employee was driving a forklift uphill that was holding bundles of beams that became unstable. With the forks on the ground, the employee attempted to move a board that was part of the load supporting the stack of bundles, causing one bundle to fall and pin the employee's leg between it and a bundle that had previously fallen. The employee was hospitalized due to a contusion to the left knee and leg.
