Incident Narrative
An employee was retrieving carpet rolls from a shelf located about nine feet above the ground while standing on a platform that was raised on the forks of a forklift in order to reach the shelf. Two of the platform brackets broke off causing the platform to tilt to one side. The employee lost balance and fell onto the concrete floor, resulting in a fractured left leg and a concussion. The employee was hospitalized.
