Incident Narrative

A mobile scaffold had fallen over and come to rest against a catwalk. A team of 10 to 12 employees were trying to right it when it moved off the catwalk, then fell to the ground. One of its crossbars struck an employee, who was trapped between the scaffold and the concrete floor. The employee was hospitalized with back injuries.
