Incident Narrative
An employee was loading a tandem dump truck with road base material. Some of the load had been dumped so the truck would not be overloaded, and material had collected under the tailgate, preventing it from closing. As the employee tried to clear away the material, the employee's right middle finger was caught between the tailgate and the tailgate latch. The fingertip was amputated at the first knuckle.
