Incident Narrative

An employee was stacking bundles of floor trusses with the forklift. When he attempted to separate two loads, controls on the forklift were bumped and caused the load to lower, pinching his right index finger between the sticker and the top bundle of floor trusses. The employee suffered a fingertip amputation.
