Incident Narrative
An employee was hauling a load of salt water to an oil well to be pumped into the well. The truck hose was connected to the well. When a valve was opened on the truck, the pressure blew the aluminum hatch off the truck and it struck the employee. The employee was hospitalized for head and shoulder injuries as well as a punctured lung.
