Incident Narrative

An employee was lancing a ladle when the lance pipe became stuck to the ladle opening. The employee then pulled on the pipe. The pipe detached from the lance and liquid steel poured out of the pipe and struck the employee's gloved right hand causing second degree burns to the palm and wrist.
