Incident Narrative

An employee was unloading a 32-inch-wide scissor lift from a trailer. A spotter told the employee to stop the lift because it was not completely lined up with the ramp. The employee stopped the lift, but it tipped and fell to the ground with the employee inside it. The employee suffered a broken left femur.
