Incident Narrative

An employee installed new control wiring for the emergency release button on a formic acid process. When the employee tested the new wiring, formic acid sprayed out of a secondary pipe and struck the employee causing chemical burns to the face, chest, right shoulder, and arm. The employee was hospitalized.
