At about 9:00 a.m. on March 31, 2021, an employee was using an overhead crane to pick up steel tubing to put it into a tube cutter machine. As the employee reached to remove a tag affixed to the tubing, his left middle fingertip was amputated by the guide gate of the tube cutter.
