Incident Narrative

An employee was preparing to pull plastic netting through an orienter (a machine that heats and stretches plastic film over a series of rollers). The employee tied a string to the netting, wrapped the string around his left hand, and began to pull the string through the machine to the end roller. The roller started, and the string tightened around his left hand. His ring and little fingertips were amputated.
