Incident Narrative

Three Kaplan Corporation employees were helping a delivery driver, who was using a pallet jack to unload a pallet of glass from a delivery truck. One of the jack's wheels hung up on uneven pavement around a manhole cover. One of the employees was trying to free the wheel when the jack's hydraulics were released and it dropped its load to the ground. The employee's right index finger was amputated at the first knuckle.
