Incident Narrative
A crew was removing an 8-inch rubber hose from inside a 2-foot diameter metal pipe. The hose was being pulled by a loader. The hose became stuck and began to stretch. The loader operator stopped and an employee went to inspect. The hose broke free and struck the employee's right shin, resulting in a broken right leg.
