On March 1, 2021, an employee was retrieving a plastic spacer used to separate rows of boxes on a pallet. A floor jack bumped into a nearby stack of pallets, and a pallet struck the employee. The employee suffered fractures to the lower leg and ankle and was hospitalized, requiring surgery.
