Incident Narrative
On February 24, 2021, an employee had just unhooked a tow strap. The employee was laying on the ground, attempting to re-insert a pin into the front of a haul truck. The truck was at an intersection that had a flagger signaling the public traffic. The flagger motioned to the public traffic and the haul truck moved forward and struck the employee that was working under the truck. The employee was hospitalized with a fractured pelvis, a detached femur, and internal injuries.
