Incident Narrative
An employee was changing a motor on a sorting line and correcting tension on the conveyor belt. The belt slipped and, as he was adjusting it, his right thumb was caught by the belt and pulley. The belt wrapped around his thumb, causing the amputation of the thumb tip.
