Incident Narrative
An employee was installing a metal air handler hood. It would not fit properly, so he wedged a 2-inch by 4-inch piece of wood in. As he was about to press it with a scissor lift, the wedge slipped out and the hood caught his left middle finger amputating it at the knuckle.
